This case study shines a light on one of the most rewarding treatments we’ve performed, where we helped a client who had suffered a traumatic injury regain not just her appearance, but also her self-esteem.
In October 2023, our client experienced a severe dog bite to her upper lip, leaving her with significant tissue damage and a loss of lip volume. The injury was so serious that she had to undergo immediate reconstructive surgery, which involved rolling up the remaining lip tissue to recreate her upper lip. While the surgery was necessary for the restoration of function, it left her with a noticeable asymmetry and volume loss in the upper lip that impacted her appearance and how she felt about herself.

Our client was looking for a subtle and natural result—she didn’t want to dramatically change her appearance but instead hoped to add some volume and hydration to her upper lip. She was particularly concerned about the loss of volume in the “white lip” area and the unevenness between the upper and lower lips. We understood the sensitivity of her situation and made sure to explain the challenges that scar tissue and previous surgery could present, but we were confident that we could help her achieve a more balanced and hydrated look.
Working closely with the client, Dr Matt carefully crafted a treatment plan tailored to her unique needs. While reconstructing the cupid’s bow or creating a perfect vermillion border might not be feasible due to the scarring, the goal was clear: restore natural volume, address asymmetry, and hydrate the lip tissue to give her a refreshed look.
For this, we chose Juvederm Volbella, a soft and smooth dermal filler that’s perfect for delicate lip treatments. The plan was to add gentle volume to the upper lip using a cannula, ensuring that the filler would integrate smoothly into the scar tissue. A small amount of filler was also used to the white lip area to help create a more harmonious appearance, while additional volume was added to both the upper and lower lips for balance.
The treatment itself was a journey, one that required patience, precision, and expertise. We began by using a 25G cannula to inject the filler, starting with a very subtle amount (0.1ml) to the upper lip. Due to the scar tissue, the process was a bit more challenging than usual, but we took our time to ensure the filler was placed carefully for the best possible result.
The treatment was carried out over several small steps: 0.2ml was added to the upper white lip for hydration, and then 0.5ml was distributed evenly across both the upper and lower lips. The delicate technique was crucial to avoid further tension on the scar tissue and to ensure the lips retained a natural look.
Though the procedure involved some moderate bleeding (which is common with lip treatments), everything went smoothly, and the results were just what the client had hoped for—a subtle increase in volume and improved symmetry, with a natural look that made her feel more like herself again.
After the treatment, we checked in with our client, and the results spoke for themselves. Her lips were fuller, more hydrated, and better balanced—without the appearance of an “overdone” look. Most importantly, the subtle enhancements gave her back the confidence she had lost after her traumatic injury.
